Every now and again I get a strange problem with the brakes and wandered if anyone knew what would or could be causing it.

When I push the brake pedal but it goes extremely hard and won't depress any further. It almost feels like some things got caught underneath the pedal. I get very little stopping power. If I release the pedal fully and then re-depress it all comes back to normal

It can be fine for miles and miles then it happens as I come up to a junction or something then its fine for miles and miles again.
It doesn't seem to matter if the brakes are hot or cold.

Check the vacuum hose is connected properly in the engine bay. It could be disengaging with engine movement.

Now how would one go about identifying the vacuum hose

comes off the rear inlet manifold I think and will head back into bulkhead

If it was a TS I could tell you. V6 - no idea.

It'll probably be a black plastic pipe ( 8mm or 10mm in diameter ) and connected to the intake box.
