I’ve seen a guide on Alfa Workshop about removing gearbox swarf but unsure of the exact procedure.

Does the gearbox have to be drained before it’s done?

The pictures show a support in place but another guide I saw doesn’t show this. What support is needed if you are just removing the end cover plate?

Is it correct that 4th gear on these boxes may not select properly or slip out either because of swarf or low gearbox oil?

A step by step and clear photos would be immensely helpful cheers.

Swarf can not do gear going out (it's syncro wear). Some broken part (big swarf) can do gear don't select properly, but it's rare. If not clear issues change oil from lowest drain point (differential). If difficult to select gear but working check cables and selector.
