Sorry I took so long, but I wanted to double check at work (where all the standards and stuff are kept):
Breathing air bottles are a 5 year test, so if the last stamp is '06 it will need doing again.
Whoever fills it for you should be able to recommend a servicing agent.
It's not an expensive process (10's of quid IIRC); and they should do a visual inspection, check the threads for wear or damage, replace O-rings if they need it, and do a hydrostatic proof test to 1.5x Working Pressure.
Test date should be stamped, or some places use a sticker and pot it in resin so it can't be tampered with.
