the blue smoke on startup is likely valve stem seals, as for the noise, that is likely to be one of two things, first off it could be the variator, does the noise go away? if it does, and the sound is very diesel like, then not to worry too much, as a noisy variator wont destroy the engine, but eventually it will loose some power. When changing the variator, have the cam belt changed too (along with the tensioners and idlers)
the other cause of this sort of noise would be a lazy valve lifter, but this is far less likely, and quite a different noise.
it may be worth getting a second hand, or re conditioned head and having it swapped if possible, it comes down to budget and / or your desire to get your hands dirty.
Autolusso can probably supply you a good second hand head - if you were planning on having the cam belt changed at any point soon, it wouldn't actually be a lot more work to swap the head at the same time..
