This happened to mine and thankfully Enzo at Autocassa saved my bella by spotting the balancer tensioner had totally disintegrated. This is how you can check it:
If you can remove or loosen enough to get a peak in through the cam belt cover at the bottom you should be able to see if the balance belt tensioner is rattling around in there. If so stop driving, don't even switch it on.
If you have to have it running and the tensioner is all broken up, you can cut the balancer belt and remove all the bits and drive it without the balancers with no problems, it may just seem a bit rough if you rev her up much. In order to replace the cam belts the balancer belt has to be replaced so you may as well get it and the tensioner done at this time whether or not they are falling apart... as I say the belt must be replaced no matter what and the tensioner is only £30 so why would you take the risk of leaving it if you don't know how old it is?
Of course if you can get a peak and it all looks solid, still get them both changed with the cam belt but look elsewhere for the rattle!
