My car did that, rattling away and sounding pretty worn-out as I drove past walls with the windows down, then at 130,000km I had a timing belt tensioner fail and a mechanic took the motor apart. There was considerable and uneven wear on the cam lobes and tappets - some cam lobes and their corresponding tappets were fine, some weren't. Apparently these motors just wear that way, especially if oil changes etc haven't been that frequent.
Now the second hand motor I had installed in the car is just as rattly. >8[
The only boxer motors with adjustable tappets were the motors of up to 1500cc in Suds, Sprints and early 33s (with probably one or two Alfa exceptions to use up old parts)... You can recognise these motors by the removable covers on the camboxes.
