I've just had the reply from the man that knows:
There were two ways, either an AP disc off of a group four Escort Mk11 with a aluminium adaptor bell or we did have some made that picked up on the standard four bolts of the early type standard disc (later discs were retained by the driveshaft bolts). It's unlikely that anything is available now but discs could probably still be made by one of the specialists such as BG Developments to order. We then made special spacers up to widen the calipers which needed longer retaining bolts which was difficult as they were an uncommon size, I think from memory 7mm cap screws. You then also needed new bridging pipes between the caliper halves and wider clips to retain the pads. So it was quite a tricky conversion with lots of special bits to be made up. On the race cars we used to take all the handbrake mechanism and self adjusting parts out and just run them as conventional calipers to avoid having to constantly adjust them. Hope this helps.
