Alfa Romeo 159 1.9Jtdm - dies after it warms up

My Alfa runs great when cold, but once the engine warms up, it looses power and eventually (after about 10miles) stops, refusing to start again. The Engine light comes on and error code P2620 Throttle Pos Output Circ/Open.

The car has the EGR blanked out, as well as the DPF replaced with a Catalytic Convertor and has been remapped accordingly.

Throttle pedal has been replaced, Crank shaft position sensor replaced too and throttle body given a proper cleanup.

Normally first thing to check is the thug noted in the fault list.
This sounds a bit strange that it works until warm, this would indicate to me that you may have a sensor issue like the MAF. The engine runs a default map until the engine reaches operating temperature at which point it relies on the sensors.
Try disconnecting the MAF and see if it runs more normally.
