Just read this thread and thought I'd add my experience.
(Although not in an Alfa)
I once took the rad cap off my Mini (original on not the BMW kind, it was 20+ years ago) when it was over heating.
I was in a rush and didn't even think of what might happen, I just wanted to get some water in the rad and carry on with my journey.
I lent under the bonnet, removed the rad cap in one swift movement and my whole hand was engulfed in super hot steam/water as it gushed out of the filler.
Within about 60 seconds the skin on my hand had ballooned and swollen to about twice it's normal size. I ended up in hospital in excruciating pain and spend 2 weeks on very strong painkillers with daily hospital visits for clean dressings.
I was lucky that I didn't end up with permanent scaring, but I'll never take a hot rad cap off again.
If you ever suspect your cap is knackered just replace it, they only cost about £5.
