'diesel particulate filter', as the name suggests, filters out the diesel particles that appear as black smoke when you put your foot down (and the rest of the time that is not so obvious!).
All modern cars have them. Unfortunately that soot builds up and has to be removed. This is done by heating to high temperatures and burning to co2 (and releasing finer particle smoke), some cars (eg Peugeot) have an expensive liquid to lower the temperature this happens at, and others (eg Alfa), injects extra diesel into the exhaust to 'burn off' the soot ( the so called 'regen' you'll see mentioned a lot!).
Unfortunately none of the methods work 100%, so the exhaust eventually gets blocked, loosing you power, reducing mpg, and resulting in a limp mode or other problems.
