Help Whats up with it now?

I have a sick 2.4 sportwagon. I think I know what's wrong with it, but would be interested in the views of others.
It is a 2007 159 2.4 200 BHP Qtronic with 62k on the clock. It had the manifold mod a couple of years ago.

The symptoms are:

Very slow off the mark, but fine once it's motoring.
Splutters and coughs just after changing up gears on gentle throttle
Takes forever to fire up when cold, then sends out a huge cloud of smoke
Smokes under hard acceleration.

There's no warning lights and it still goes like the clappers.

Is it most likely to be the EGR, MAF, manifold, or something else?

what colour is the smoke on startup?
if it's sooty eg overfuel, this is a symptom of a dead/dying MAF

It's definately grey smelly diesel smoke on start up
I was guessing EGR, but I could do with some help!

I would say EGR. The MAF sensor only really acts to diagnose any faults with the EGR and rarely goes wrong on diesels.
