I do tend to use injector cleaner every three to four tanks.
When the car has suffered coil pack failures previously they developed intermittently and became more prominent / regular as time went on. They also reported themselves as a specific cylinder failure.
I would expect a failed injector to be a constant miss-fire. At idle there is no miss and if I rev the car slowly it's fine too. It will still sit happily on cruise at motorway speed, even in fourth gear at +4000rpm, so the injectors seem to be atomizing and flowing.
The current problem seems more like an ignition issue under load, so like a failed coil pack. I've had all the plugs out (Bosch Premium) and they are all fine, I've also replaced each coil pack in turn with a spare and seen no improvement.
It still seems odd that the problem occurred immediately (within half a mile) of brimming the tank. What also has me thinking fuel is the odd smell when I was filling up, it seemed to smell a bit like diesel, I remember looking to ensure I had the correct nozzle as I do often fuel the Mrs's Diesel. At the end of my shift run I'll hopefully have used almost all this fuel, I do roughly 50 miles a day, and I'll see if a full tank of V-power sorts it out.
Looks like I had probably also take my mate up on the second hand set of JTS Kats he offered me previously..... They were already on their last legs and I'm sure this won't be helping..
