Re: What would you do?

Give it back asap. with only 44k and major engine problem, I'd say it's been abused / not serviced right. High risk of future problems and who's to say the warrantee will cover everything down the track. Wifey overheated her old BMW E30 about a year ago and there's still things going wrong with it that are related to the overheating.

Ditch it before it bites you in the bum. You'll not regret returning it and getting another car (Alfa or different) but you will regret getting it repaired if it costs a heap later on down the road.

Re: What would you do?

I'd agree with the idea of only buying from an alfa dealer or an obvious enthusiast. They're the ones that are familiar with all the problems and noises they make and are generally not going to flog you a flogged Alfa!

If you get the engine rebuilt under warranty then get it done by an Alfa specialist or dealer, then you'll know it's been done properly. It's pretty easy to check over the rest of the car to see if anything else has been neglected (although gearbox is a bit of an unknown!) i.e suspension and bodywork so if the engine was to be rebuilt I wouldn't worry about the car failing for any other reason any more than any other 156.
