...assuming all the fixings / bolts are not loose the hinges are normally (as far as I know) sleeved with a DU type bearing which is basically a wrapped piece of metal with copper / self lubrication properties (has to cope with water / rain / salt etc.). If this DU bearing is worn it may just need to be replaced (best to see if your dealer can supply a new bearing) - This is a job I've not had to tackle myself but I would think it straight forward, depending how the hinge pin is assembled. It would probably be best to remove the door completely to avoid bodywork damage. You've not said if it's a 156 or 147 but I think this type of construction is fairly common.
This is just the little I know...maybe someone has more direct experience of door hinge problems.
