Get to drive to someone familiar with Alfas.A rattling noise will be heard then the noise will be located at the bottom end or top end and advice given (iknow it could be small ends ! in the middle). Try and find out its history cos a recent cambelt snap could have shock loaded the half bearings, i.e big ends.It could be the variator cos when they start to get noisey its usually on startup but if left they can produce all sorts of top end sounds.

i get rattaling from about 1k to 2k and i've been told its the heat shield. they are prone to cracking and when they crack they tend to vibrate and rattle.
