Help carbon build up

Any thoughts about carbon build up in diesel engine.
I replaced EGR valve in June after lost power completely.
Then last week the same problem again and had to have the intake manifold replaced as it was actually clogged with soot with intake flaps stuck. Garage never saw a problem as bad.
What could have caused the problem . . . poor quality diesel . . . not getting up into the high revs? Alfa is 3 years old with 85k on the clock.
