It is just two cables, one does the sideways movement on selection, the other does sideways. They are connected to the linkage in the gearbox with ball and socket joints, so when they wear out a bit, the cables aren't quite pushing the selector forks to the right place hence the notchy gearchange. I haven't done it on the Alfa, but I changed them on my Fiat Coupe which is a very similar set up. It massively improved things, but you have to take off the cat/centre section of exhaust and then the exhaust tunnel to access the bottom of the gearshifter, then it's just a matter of popping off the sockets and replacing with the new ones, then feed it through to the engine bay and do the same there with the selector forks. It's not difficult to do as a DIY job.
