It's probably the water temperature switch/sensor in the radiator, sticking in the "on" position and completing the circuit.
The only way to test it is to connect one lead from a bulb to the switch. Connect the other lead from the bulb to the battery (+) and connect the other prong on the switch to the battery (-).
If the switch is okay (cold engine) the circuit should be open/bulb off. If the switch is sticking on, the circuit will be closed and the bulb will come on.
Change the radiator switch if it's sticking on. They're small so only going to be a few quids.
Until you solve it, you can run some wires from the temp sensor switch to a manual switch inside the car and back to the live feed. Then you have to switch the fan on and off manually until you sort it.
